Care 1st and Citrus Valley Name Changes
Effective January 1st, 2019 the following Insurance / Medical Groups have changed their names:
1. Care 1st Insurance Plan is now Blue Shield of California Promise Health Plan (Blue Shield Promise).
Benefit coverage for these patients have changed as well. Below is the Evidence of Coverage information provided by Blue Shield Promise.
Advantage Optimum Plan (HMO) – LA, Orange Counties
$1,500.00 limit for HAs per year
Coordinated Choice plan (HMO) – La, Orange, San Bernardino, Riverside Counties
$2,000.00 limit for HAs per year
Total Dual Plan (HMO SNP) – LA, Orange, San Bernardino Counties
$1,500.00 limit for HAs per year
We are currently updating the IVFs, Quote Sheets, and Sycle to reflect the new Plan name.
2. Citrus Valley Health Partners (Citrus Valley Medical Center, Citrus Valley IPA) have changed their name to EMANATE Health Medical Center.
At this time, we are unaware if any benefit coverage has changed for these members.
We are currently updating the IVFs, Quote Sheets, and Sycle to reflect the new Plan name.
